About CCACs

CCACs are local organizations that can help you access government-funded home care services and long-term care homes. We also help people to navigate the array of community support and health agencies in our communities.  The Bringing Health Care Home brochure describes the services that the CCAC provides.

CCACs are funded and regulated by the Ministry of Health and Long-Term Care and coordinate a variety of health services to maintain an individual's health, independence and quality of life.

CCAC In-Home Services

If eligible, the health care services you may receive include:

  • Nursing
  • Personal support (help with bathing, dressing, etc.)
  • Physiotherapy
  • Occupational therapy
  • Speech-language therapy
  • Social work
  • Nutritional counseling
  • Medical supplies and equipment

Specialized Services

CCACs also offer specialized programs geared to specific health needs:

  • Acquired brain injury
  • Convalescent Care (Short-term care in a Long-Term Care Home)
  • Respite care (in-home or short-stay at a Long-Term Care Home) that provides a break for the caregiver
  • Child and family services
  • School Health Support
  • Mental Health
  • Palliative care (care at the end of life)

Referrals to Service

Anyone can make a referral to a CCAC - you, a family member, caregiver, friend, physician or other health care professional.

When you contact a CCAC you will likely speak with a Case Manager or Care Coordinator.

Case Managers can help you to plan and make informed choices.  Case Managers work in partnership with clients, families, care providers, hospitals, long-term care homes and others, to assist with making the right care available.

Long-Term Care Homes

When living independently is no longer possible we coordinate applications to Long-Term Care Homes in the area and across Ontario. Long-Term Care Homes are funded and regulated by the Ministry and Health and Long-Term Care.

CCAC staff can assist you by:

  • Providing information about long-term care homes
  • Determining suitability and eligibility for placement
  • Providing assistance in the application process.

Health Care Connect

Health Care Connect is a program launched by the Ministry of Health and Long-Term Care to help Ontarians without a family health care provider find one.  The program refers people without a regular family health care provider to physicians and nurse practitioners who are accepting new patients in their community.  If you don’t have a provider you can register for the Health Care Connect program by calling 1-800-445-1822.  For more information CLICK HERE and for frequently asked questions, CLICK HERE.
